Spring 2012 Registration

Online registration closes Friday, February 10th, 2012

• Medical Release form (if new player to DSA)
• DSA Membership Form
• NJYS Membership form (travel players only)
• Player and Parent conduct form (if new player to DSA)
• Copy of Birth Certificate
• Forms can be found and download on our Registration page

• Eligible participants must be between the ages of U-3 and U-9 (those born between August 1, 2002 - July 31, 2009).
• All players receive Team Shirt.
• All games played on Saturdays.
• The season begins in March - start date will be posted soon!
• Parents/guardians will need to provide shorts, socks, shin guards and cleats.

• Recreation = $70.00
• U8 to U10 Travel = $100.00 (D.O.B - August 1, 2001 through July 31, 2004)
• U11 and Up Travel = $110.00 (born prior to July 31, 2001)
• Late registration fee  - On-line only (2/06/12 thru 2/10/12)= $25.00
• NO Registrations will be accepted after 2/10/12
